Title: Ban appeal bug.

For the ban appeal section when you click on the link it provides this pops up, please fix it.


 Fatal error: require_once() [function.require]: Failed opening required '/var/www/Sources/CustomForm.php' (include_path='.:/usr/local/lsws/lsphp5/lib/php') in /var/www/index.php on line 370

Forms aren't being used anymore (from what I've heard) so you'll need to wait for someone to take down the clicky-link-form-button and remake the format and allow new topics
