CWU Applications are broke

The title says it all, It sends me to

"Fatal error: require_once() [function.require]: Failed opening required '/var/www/Sources/CustomForm.php' (include_path='.:/usr/local/lsws/lsphp5/lib/php') in /var/www/index.php on line 370"
Is anyone else getting this?

It's still using the forms thing which don't work anymore, they need the forms removed and the old format+posting to be brought back
